Survey Instructions:

Not sure how to get started?  Here's a easy steps....

If you already have a code, go to the Survey System and either:

  1.  Take the Survey using the code you have, or
  2. Check/Update your organization information by logging in.

If you need to create a code for your organization:

  1. Go to the Survey System and click on "REGISTRATION".
  2. Select "Create a New Registration"
  3. Read the Instructions and enter all of the information about your organization.
  4. In the "Referral Code" - enter the code for the Approved Vendor that referred you to this site.  If you don't know what this means, leave it blank!!
  5. In the "Default Survey" - enter the code for the survey you want everyone to use.  If you leave this blank, you can take any of the public surveys!
  6. In the "Website" field - enter the URL for the web location you want all of your people who use this site to go after they've completed the survey.  If you leave this blank, they will go to our homepage.
  7.  In the "OrgCode" field create a unique name for your organization.  Try something that your people will remember - everyone in your organization who comes here to take the survey must enter this code!!

That's it - you're done and ready to go!

If you want to Update any of this registration information, return to the registration system and click on "UPDATE" from the login page.  You will be required to enter your OrgCode and identify yourself with the same email address you used to register originally.  Then you can update any of the information you posted.

It's that simple - and you're on your way.......
